Welcome Parents and Students to DATA @Adams Athletics!

All 7th or 8th-grade students that are interested in participating in the athletics program here at Adams, must have a physical on file and complete the online forms.  Students will not be able to try out for sports without completing these steps.

You will find a physical form online HERE. 

The UIL forms that must be completed can be found and submitted HERE.

Directions:

1. Click on "Proceed To Online Forms"

2. Click on "Continue as a guest"

3. Click on the blue Emergency Info Form and complete the form

4. Click on the blue UIL/Athletc Participation Form and complete the form

All the physical forms and the online forms must be completed before you student can try out for the sport.
