    4. Official Service Records from previous district(s) (we cannot accept copies-Due by Oct. 1)
    5. I-9 Printed and Completed (only page 7 of 9)
    6. W-4 Printed and Completed
    7. View "New Employee Information Series" Videos and bring this completed form.

    In Addition, Alternative Certified Program Teachers Also Bring:
    1. Acceptance Letter from Alternative Certification Program
    2. Statement of Eligibility from Alternative Certification Program
    *Be sure you have Paid for your Probationary Credentials (contact your program)

    Paraprofessional Staff Must Bring:
    1. 2 forms of ID AND one copy of your ID
    2. Social Security Card (can also be one of your forms of ID)
    3. Official transcripts with degree awarded/total credit hours, or a copy of high school diploma/GED
    4. Service record from former school district, if applicable
    5. Completed I-9 (page 7 of 9 only)
