Clinic

Medication: If your child will need to have any medications administered at school, (even over-the-counter medications), you will need to provide the clinic with a medication permission order (generic versions are available from your doctor). You may stop by the clinic and get one from me. If I am out, Mrs. Carrie Hargrove or Susie Chavera can provide you with one. School medication consent MUST signed by both you and your child’s doctor. If the medication is a prescription, it will need to be in the original packaging with the prescription label attached (no pills in baggies, mislabeled/expired pill bottles, inhalers without the box bearing the prescription label, etc).

Important: Please keep sick kids home: Students must be 24 hours free of symptoms (fever, 100.5 F or higher, vomiting, or diarrhea). Kids DON’T always need to see the doctor, but they DO need to stay home for care.
